We multiply per-cycle line items by your selected number of cycles, scale by regional cost differential, and apply a flat insurance reduction. The model does not account for: clinic-specific package deals, multi-cycle discounts (often 15–25% off two or three pre-paid cycles), shared-risk / refund programs, military or grant funding (Resolve, Baby Quest, BCRFA), or out-of-pocket maximums on your plan.
State mandates are a maze. Even within "mandate to cover" states, employer plans that are ERISA-self-insured (most large employers) are exempt from state law.
